Value-Based Care Model:
- Physician Role: As the "quarterback" or "captain" of the team, the physician will provide holistic care, collaborating with a care team that includes a clinical pharmacist, nurse, health coach, behavioral health specialist, and medical assistant.
- Preferred Experience: Clinical experience in family medicine, internal medicine, or ambulatory care.
- Qualifications: MD/DO, Board Certified/Board Eligible (preferably early-career physicians).
- Schedule: 8:00 AM – 5:30 PM, Monday to Friday (flexible with 4x10 hour shifts). Last appointment at 4:40 PM.
- EMR: NextGen.
- No On-Call Duties.
- Patient Visits: Average of 12-15 patients per day, with annual wellness visits taking 1 hour. Complex patients are seen 6-8 times per year.
- In-Office Procedures: Simple suturing, laceration repair, diabetic foot exams, IUD insertion (a plus).
- Point-of-Care Ultrasound: Available onsite.
- Patient Panel: Max panel size of 1,250 patients.
- Patient Demographics: Primarily seniors, with some pediatric patients.
- Independent Primary Care Center: Not affiliated with a hospital.
- Insurance: Medicare through Anthem and CMS.
- Referrals: PicassoMD for referrals.
- Partnerships: LabCorp, Urgent Care Center next door.
Compensation & Benefits:
- Salary Range: $230,000 – $250,000 base, plus 10% bonus potential.
