Lafayette, Louisiana
Primary Care Plus is seeking qualified Internal Medicine and Family Practice physicians to join our expanding practice in Algiers, Louisiana. Our clinics are part of a clinically integrated network providing enhanced care to Medicare Advantage, Quality Plan, and ACO members.
Responsibilities

Develop and implement patient care plans

Provide patient and family education and counseling

Order and interpret laboratory and diagnostic studies

Prescribe medications and treatments

Refer patients for specialist consultations when needed

Thorough documentation of patient progress notes and summaries

Participate in patient care rounds and conferences

Collaborate with a multidisciplinary team to meet patient care needs
Qualifications
-
Doctor of Medicine (M.D.), Doctor of Osteopathy (D.O.), or M.B.B.S
-
Board certification or eligibility in Family or Internal Medicine

Active, unrestricted medical license in Louisiana

Valid DEA license
Preferred Qualifications

Certification in Basic Cardiac Life Support

Board certification in Internal Medicine or Family Medicine

1+ year clinical experience in geriatric, adult, or family practice setting

We value collaboration among physicians, APRNs, RN Care Managers, and a Quality Improvement team to coordinate care for patients in both outpatient and inpatient settings.
We offer a supportive environment with consistent feedback and focus on patient outcomes rather than productivity or RVUs. Our physicians typically manage panels of 600-800 patients and have 30-60 minute appointment slots.
