The Role
We are seeking a full-time physician trained in Geriatrics, Internal Medicine, or Family Medicine to serve as the primary care provider for medically complex seniors in the PACE model. This is a collaborative, patient-centered position where you’ll work closely with a multidisciplinary team to coordinate and deliver comprehensive care.
• Setting: Outpatient, nursing home, and home care coordination
• Schedule: Full-time; no call or weekends required
• Team Collaboration: Daily IDT meetings, integrated care planning
• Responsibilities Include:
o Primary care visits and chronic condition management
o Acute illness evaluations
o Nursing home rounding and care coordination
o Integration of specialty care and hospital oversight
o Quality improvement through data review and clinical collaboration

• Base Salary: $208,000–$241,000

Requirements:
• MD/DO with board certification in Geriatrics, Internal Medicine, or Family Medicine
• Minimum 1 year experience working with frail or elderly populations
• Must be interested in permanent employment
