About the Location
This opportunity is based in Northern Indiana, where community care meets leadership in an innovative Program of All-Inclusive Care for the Elderly (PACE). Help shape and lead a patient-centered model focused on keeping frail seniors safe and supported in their homes.

The Role
We are seeking a Board-Certified Internal Medicine or Family Medicine physician with fellowship training or CAQ in Geriatrics to serve as Medical Director for a well-established PACE program. This leadership position combines direct clinical care with programmatic oversight.
• Primary Responsibilities:
o Medical oversight of care delivery and clinical outcomes
o Supervision of Primary Care Providers and APPs
o Active role in QAPI program, policy development, and leadership initiatives
o Conduct new participant physicals and reassessments
o Participate in Intake and Interdisciplinary Team (IDT) decisions
o On-call rotation as needed
• Skills Needed:
o Excellent geriatric assessment and chronic care management
o Familiarity with EMR systems and Microsoft Office tools
o Effective team leadership and interdepartmental communication
o Flexibility and compassion in a community-based care environment
• Travel: Periodic local travel required – must have valid license, insurance, and reliable transportation

• Salary Range: $231,000–$280,000 (based on experience)

Requirements:
• Board Certified in Internal Medicine or Family Medicine

• Minimum 1 year experience with frail/elderly populations
