Location: Henrietta Johnson Medical Center Department: Behavioral Health Department Job Title: Mental Health Therapist (LCSW) The Mental Health Therapist – Licensed Clinical Social Worker reports directly to the Psychiatric Mental Health Nurse Practitioner. This role involves providing brief counseling and psychotherapy to adults, children, adolescents, and their families to enhance psychosocial functioning. As part of a multidisciplinary team, the therapist collaborates with medical providers to deliver services through an integrated care model. This position is available on a full or part-time basis, working onsite Monday through Friday. Essential Duties and Responsibilities: Provide individual, family, and group mental health counseling for patients. Deliver individual and small group counseling using evidence-based or best practice models. Complete required documentation promptly and efficiently, adhering to organizational policy. Collaborate with the HJMC interdisciplinary team to provide mental health care recommendations and participate in multi-disciplinary team meetings with outside agencies. Consult with primary care providers, psychiatrists, substance abuse counselors, care coordinators, and other mental health professionals as needed. Make appropriate referrals based on clinical judgment, providing patients with at least two care options. Maintain professional behavior and relationships with HJMC staff and referral resources. Engage in professional development activities and utilize clinical/educational research to enhance therapy services. Adhere to HJMC policies and procedures. Identify and diagnose psychological, emotional, or behavioral issues using information from interviews, tests, records, consultations, and reference materials. Support medication therapy prescribed by PCPs. Attend staff and other required meetings. Perform general administrative/office duties as needed. Complete and maintain client-related paperwork, including federal- and state-mandated forms. Provide crisis intervention, emotional support, resources, and referrals. Participate in caseload consultation with the psychiatric consultant and communicate treatment recommendations to medical providers. Screen and assess patients for common mental health and substance abuse disorders. Facilitate patient engagement and follow-up care. Utilize registry and tracking systems to collect and collate data, ensuring adherence to standards for administration, case recording, and written reports. Document patient progress and treatment recommendations in EHR and other systems for sharing with medical providers, psychiatric consultants, and other treating providers. Facilitate treatment plan changes for patients who are not progressing as expected, in consultation with medical providers and psychiatric consultants. Develop relapse prevention self-management plans with patients who have achieved their treatment goals and are nearing discharge. Support the mental and physical health care of patients on an assigned caseload. Coordinate care closely with patients' medical providers and other mental health providers as needed. Provide or facilitate referrals to evidence-based psychosocial treatments as clinically indicated. Qualifications: Licensed Clinical Social Worker with up-to-date Delaware Licensure Excellent knowledge of behavioral medicine and evidence-based treatments Ability to work effectively within an interdisciplinary primary care team Strong written and verbal communication skills Good knowledge of psychopharmacology Ability to design and implement clinical pathways and protocols for individual and group treatment Working Conditions: This is an exempt position requiring a minimum of 40 hours per week onsite.
