Position Overview/Responsibilities for the Case Management Assistant:

- Support care coordination and discharge planning for ambulatory patients
- Collaborate with Case Managers, Social Workers, and external vendors to ensure a smooth transition of care (TOC) and timely discharge
- Facilitate referrals, authorizations, and transfers to post-acute facilities
- Participate in daily rounds, team huddles, and department meetings
- Maintain accurate, up-to-date records in the electronic medical record system

Required Skills for Case Management Assistant:

- Minimum of 1 year of relevant experience
- Strong communication, interpersonal, and time management skills
- Ability to prioritize tasks and work independently in a fast-paced environment
- Proficiency in PC applications and word processing
- Familiarity with electronic medical records (EMRs)
