Position: Care NavigatorLocation: Progressive Health of Delaware
Progressive Health of Delaware is looking for a World-Class Care Navigator to join our Primary Care team. Keep your own panel of patients healthy through lifestyle medicine!
The Care Navigator will be part of our care management program, connecting with patients by telephone, the electronic medical record, and other means to coordinate their healthcare across the spectrum. Activities would include contacting patients to schedule appointments and lab work, coordinating referrals to specialists and community resources and supporting patients on their personal health goals. The ideal candidate is an experienced case manager, certified medical assistant or LPN. Primary Care or chronic disease management experience is a plus.
